Our Constituent Card provides you with the opportunity for first hand trips to Chicago's Museums, Zoo, Gardens, and Aquariums. With over 15 million visitors per year, these remarkable institutions offer visitors priceless opportunities to explore and enjoy the arts, sciences, nature, and culture.

1. Card only grants holder of the card for ONE DAY USE.
2. We require at least 2 weeks notice for the request to be fulfilled.
3. General Admission Tickets Only- no event admission or parking is included.
4. You must state how many ADULTS and CHILDREN are attending. No more than four people can attend for free.
5. The person filling out this form must be in attendance and show ID at the entrance.

18. The DuSable Museum and Shedd Aquarium require the physical Museum Pass to be picked up from our office. All other museums allow use of the electronic version of the pass, which our office will email to you. If you choose the DuSable Museum or Shedd Aquarium, you understand that you MUST pick up the Museum Pass from our office, located at 221 Springfield Ave, Unit 3, Joliet the day before or morning of your visit. It must be returned by the next day, during regular business hours, Monday through Friday, 9a-5p.
